Collaborative Community Connections
Collaborative Community Connections (C3) is a housing support program that works in collaboration with housing and employment service providers to create a well-connected network of services for homeless individuals. C3 helps people reach a level of self-sufficiency consistent with their goals and capabilities for housing, employment and social well-being. Our Outreach Triage Teams focuses on strategies that develops a specialized understanding of circumstances and needs of each individual. The teams consist of local police, social workers, community organization, churches, city officials and everyday citizens.
Training sessions are held each Monday to prepare volunteers to join a team of 3-5 volunteers led by outreach leaders. Teams target Hotspots and work within Portsmouth to reach and assist homeless and hungry people and help those who want housing. Schedules for each team are determined by the team and occur anytime between 8 a.m. and 8 a.m. every day of the week.
